2022-2023 Covid-19 Policy and Waiver

Any student or tutor who has a fever or cold symptoms must stay home from in-person classes until symptoms have passed and the fever has resolved for 24 hours without fever-reducing medications.

If a student or tutor receives a positive result from a COVID-19 test and you want to share your results with SPR, please contact Mrs. Sam Fatzinger.

If a student or tutor tests positive for COVID-19 and notifies SPR, the Board will notify all families, while guarding the privacy of others, in a timely fashion so that parents can decide if they want their students to meet in-person during that time.

Any student or tutor who tests positive for COVID-19 must stay home for 5 days from when first symptoms appeared, and may only return when there has been no fever for 24 hours without using medications and symptoms are improving.

All assignments will be posted to Jupiter so that learning can continue at home.

In the event of a positive COVID-19 event, the classrooms, restrooms, and common areas will be thoroughly cleaned before students return to in-person classes.

All families must have a signed COVID-19 waiver form on file (see below).

  By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my family (anyone participating in the tutorial), may be exposed to or infected by COVID-19 while on site at Saint Peter the Rock Home School Tutorial (SPR), and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at SPR may result from the actions, omissions, or negligence of myself and others, including, but not limited to, the SPR Board of Directors and tutors, fellow SPR members and their families, and any onsite contractors or employees of our host facility, Sacred Heart Church. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my family, my guests, or myself including, but not limited to, personal injury, disability, and death. Further, I release SPR of any responsibility related to illness, injury, disability, death, damage, loss, claim, liability, or expense, of any kind, that I or my family may experience or incur in connection with my family’s attendance at SPR or participation in SPR activities.
  
  On my behalf, and on behalf of my family, I hereby release, covenant not to sue, discharge, and hold harmless the SPR Board of Directors, Sacred Heart Church, and their associated employees, agents, and representatives, including tutors, of and from all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the SPR Board of Directors, Sacred Heart Church, tutors, and associated employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in SPR activities.
  
  By signing this agreement, I acknowledge that each family member of SPR has to take personal responsibility to protect themselves, and their family, including, but not limited to, social distancing, wearing appropriate protective gear, washing hands, and following other CDC recommended guidelines.
  
  The parties agree that this waiver may be electronically signed. The parties agree that the electronic signature appearing on this agreement is the same as a handwritten signature for purposes of validity, enforceability, and admissibility.
